Here's some options for you:
http://www.psionicsoundworks.com/
http://www.doepfer.de/R2M.htm
http://www.kilpatrickaudio.com/?p=striptease
I have the Doepfer (which comes in standalone and modular versions) and it's great.
I've also got the Kilpatrick, but only used it with their killswitch pedal so far. The plan is to grab a Doepfer footswitch module and use it as a mini ribbon for my A-100 and Moog gear.
Haven't tried the Walking Stick, but I hear it's great if you can get one. It's on my shopping list.

I've got a customized v3 model (one of the 'newer' ones) which is to say that it has two CVs each with an onboard attenuator. One CV output is for position on the 500mm ribbon, the other is the amount of pressure (how hard you push onto the Ribbon).
If you look at the Psionic Soundworks web site, you'll note that they are making them self-powered now (originally they were passive and leveraged the voltage coming from ring of the device that is being controlled such as the Fooger or Synth). Now it comes with an AC adapter capable of driving voltage upwards of 10V or so. Either is old school, which is to say that there is no CPU controller or fancy circuitry such as what you'll find on a Doepfer.
The finish is very nice (mine is custom stained to match my Whitewash Old School) and the components high quality (they'll even provide knobs to match your synth).

If you have any special requests, Joe Sperduti will be (time permitting) happy to work with you to suit your needs. There are some great demos on YouTube which you may have already seen:

$249 as pictured on the link above. I paid a bit more for mine (it has a 3rd 1/4" lead wired to a momentary switch and a toggle switch to reduce the sensitivity of the pressure CV [and also the finish I mentioned].
I think the Doepfer is $400 all together and obviously offers all sorts of bells and whistles but I've got enough complexity in the rest of my rig and this fits nicely.

It would be awesome if Moog released an updated 1150 Ribbon controller that could be used with the vintage Model D Minimoog as well as the newer Moogs.

That would be great. Would need to output both CV and midi.
