Actually "Switched on Bach" came out in 1968 (your site says 1969).
Here is maybe the first or second Moog LP from Japan

The release date was 1972, the 2000 date mentioned was when the CD reissue came out.
The Korean album you have, "Nonstop Moog Moog" is interesting since there is very little info about synth pop from other parts of Asia besides Japan.

That's a great collection of covers - some amazing ones there. I didn't see Gershom Kingsley's God is a Moog, though - unless I missed it?
I think it's good that you have both versions of the cover for Switched on Bach - what I think Wendy Carlos would probably call the silly one and the sensible one. It's also worth noting the odd fact that the original versions of the album didn't mention her name (or to be precise, Walter Carlos) and gave the impression the Moog modular did everything by itself without human intervention - which is especially ironic considering the work involved in keeping the damn thing in tune with its early 901 oscillators, never mind playing it.

Interesting about the credit on the cover. I never noticed that though I've never compared different pressings over the years. I assume Carlos was mentioned on the back. What about the spine? The LP I have was a later one and has the Benjamin Folkman co-credit that's since been removed.
I guess another interesting piece of trivia is that's CBS Record's Moog on the cover, not Carlos'. I'm sure some people bought that model (IIIP) after hearing the album. I think Tomita did some years later

Yes, it credits Walter Carlos "with the assistance of Benjamin Folkman" on the back cover. The spine is the same as the front cover.
And yes, as you say, that's not the Moog that was used on the album. If they'd taken photos of the original, in Carlos' presence, I suppose, they would never have been able to take the photo they used.
It's an interesting historical document for attitudes to electronic music, though - or should that be "hysterical document"?
I'm not sure which cover failure is least excusable in Carlos' career - this early one or the one for Tales of Heaven and Hell, which at least appeared on the shelves as it should be, despite the fact that the initial copy returned from the printer's had Carlos' bright orange flames rendered in a weird shade of green. Given the massive efforts she put in with the music, I'm sure she could have done without the additional hassle that record company and printers gave her with some of the covers.
