Theremin to Midi

Theremin and MIDI are not very compatible; MIDI was designed for a typical keyboard layout, and the theremin is a monophonic, gliding pitch with a wide range. If there is such a thing, it will depend on pitch bend to track the glide, and is thus dependent on the target synth for it's success. It will also depend on your playing style.

However, there may be a possibility that a continous controller or NRPN (non-registered parameter number) can do the trick. OSC (open sound control) may be worth looking into too.

You will simply need something that can track pitch (pitch-to-midi, voice-to-midi) and convert that into suitable data. Some theremins also output CV which can be digitized.

This video (starting at 3:33) has a Moog Etherwave Plus with its Audio Out going to a G2M Guitar to MIDI Controller, which then controls a Roland synth via MIDI:
