Replacement parts for VCF Cutoff freq

Hey,
I got a problem with the pot(or the potentiometer?) of VCF Cutoff frequency and the LFO Rate pot. It should be replaced with the new one. I'm now trying to do it by myself.
Does Anyone know the name or the number of the replacement parts for them?
I'm not familiar with electronic parts. So I'm so glad if someone helps me.
