I have only seen that one. It was auctioned off on Ebay. Can try contacting the seller as the auction page is still there.
http://www.ebay.com/itm/Moog-907-Bandpa ... 7675.l2557Looks to be an early version of the 907, probably 1964/65. Only those very early modules used the RCA type sockets and a male 3 pin Cinch on the back for power. Eric Siday had that early construction in his 1964 system. Bob probably sold and produced a few modules with that configuration before moving on to the more familiar 1/4" sockets and 22 pin edge card connectors. The October 1965 catalog lists the 907 as a "fixed filter bank" with ten fixed filter sections, and that is the standard 907 configuration. So this five band version would most likely have been produced before October 1965.
Cloning that module from the photos would be the best way to hear what it sounded like. Uses resistor–capacitor filters and not the inductor coils the later standard 907 used. So the tone would be quite different.
Unknown how many were made. Have seen claims of odd modules being the only one produced then later seeing the same module in a different system.
