It's fairly straight forward although I don't remember it off the top of my head. You basically ignore the bank numbers and think of it as 256 presets. From Ableton you may have to treat it as 2 banks of 128. If I remember to look when I get home in a few hours, I'll get you exact details.

You have the correct number of presets. After 9/6 the rest are intentionally left blank for you to use as "user" patches.

I experience a similar issue. The auto update erase the firmware but when I turned the power back on (with pitchwheel all the way up). It would act like it was installing the new firmware but almost immediately return with "device not found". It found the device fine to erase the firmware....

Is Moog really keeping these serial numbers consecutive. 4000 built in about a year seems impossible given the size of the space/staff.

It might help to tell us what kind of MIDI messages you are trying to send/receive and what devices you are trying to communicate with. When MIDI data is received by the Sub37 a red light comes on.
