Why should there be incorrect MIDI note-off messages?

Ground loops create an audible buzz at 60 or 50 Hz (depends on mains system). I never ever heard about this causing a bad tuning on analog synths. For a steady pitch, you need to have a well defined constant voltage at the VCO. And so my guess is a faulty PSU of your Moog or a massive voltage fluctu...

There are reliability issues with the SubPhatty rotary switches. I had to exchange them on my SubPhatty, when two became unusable. Maybe the very same part is used on the Sub37?

There is a period of TI switching chips known to be faulty on Voyagers. As there are many of these chips used on the analog board, it is only recommended for experienced electronic soldering musicians to take care of this at their own risk. Maybe you assume, the Subsequent is sending audio over USB. Most synths don't do this. They use USB for MIDI. So audio cables and a mixer/audiointerface and so on is needed. Maybe try using the headphones connected to the Subsequent.

But if you know this, there is maybe just a setting not done right.
