My Sub Phatty Oscillator 1 Octave Knob is no longer responding. Was this determined to be a hardware or firmware problem? I am running firmware 2.03, I believe.

If every sixth key is missing anywhere you play on the keyboard, the problem is not the keyboard. You can find a service manual online that will help diagnose which voice is problematic. I have been able to control the Werkstatt with a Beatstep, Roland MPU-101 and Pittsburgh Modular MIDI3, both with and without the CV Expander. It does seem that the different gates can affect the Werkstatt output level, though.

When running external sounds thru the Phatty, you must open the gate as described in the posts above, AND the VCA sustain level should be set to maximum.

I am working on a composition with the Werkstatt where I am playing the Werkstatt button-board, controlling the filter cutoff with a Moog 1120 footpedal controller and running it through an MF-104M Analog Delay. This is No Toy! It may be limited, but it sounds great and is quite playable.

My Roland MPU-101 MIDI to CV Convertor manual has A442 in "Mid" transposition at 2.75 V. MIDI note 48 is approximately 1 V. The MPU-101 will transpose up and down one octave. The maximum voltage range shown for MIDI notes 0-127 is -3 V to +7 V.

The Rogue has a similar issue when using CV input. I use a signal processor (dotcom) to offset the CV signal and bring things back in tune.

The Werkstatt envelope is internal only, unless you modify the unit. I am using the Werkstatt with the Beatstep too. Have you tried deleting any Sub Phatty objects in Audio MIDI Setup utility? I would delete any I find and then plug the Sub Phatty back in via USB.
